Surgery Patient Information
Scheduling Surgery and Procedures
A lead-time of 15 days before surgery is requested. This allows
time to meet the requirements of your insurance, and to find time
that is convenient to you your doctor. For some insurance companies
this may need to be extended. If you are a patient of Dr. Jones
or Dr. Lee, please call and ask Cheryl at 713-773-2273 ext. 123
to schedule your surgery. Cheryl will assist you in picking a date
and making hospital arrangements.
Please contact your insurance company to confirm whether a second
opinion is required.
Consent forms must be signed prior to scheduling surgery.
